- We call you back with the results at the time you indicated. This call is a clinical and benefits conversation, not a sales pitch. You’ll get a clear breakdown of your coverage, and our team can answer any questions about programs, scheduling, or the admissions process.
- You decide what to do next. That might be scheduling an admission, getting a clinical assessment, taking time to think about it, or deciding Maverick isn’t the right fit. All of those are valid responses — we don’t push.

Maverick is in-network with most major PPO insurance carriers, including:

We also work with many other PPO carriers on an out-of-network basis, and many of those plans still provide substantial coverage for our level of care.

Questions about insurance coverage
Will my insurance actually cover rehab?
For most PPO plans: yes, substantially. The Mental Health Parity and Addiction Equity Act requires most insurance plans to cover substance use and mental health treatment at the same level they cover other medical conditions. What varies is the specific coverage — your deductible, your coinsurance percentage, whether prior authorization is required, and how many treatment days are covered before any review.
What does "in-network" mean for me?
In-network means Maverick has a negotiated rate with your insurance carrier, which typically results in lower out-of-pocket costs for you. Out-of-network coverage can still pay a significant portion of treatment costs, but the math is different and depends on your plan’s out-of-network benefits.
Will I have to pay anything?
Almost certainly some amount — most plans have a deductible (the amount you pay before coverage kicks in) and coinsurance (a percentage you pay after the deductible). Your exact out-of-pocket cost depends on your specific plan and how much of your benefits you’ve already used this year. Our admissions team will give you an estimate, in dollars, before you commit to anything.
Does insurance cover PHP and IOP equally?
Coverage levels are usually similar across both PHP and IOP, though IOP often costs less out-of-pocket because it involves fewer treatment hours per week. The same applies to standard outpatient.
Will my employer find out I'm verifying coverage?
No. Insurance verification is confidential under HIPAA and (for substance use specifically) under 42 CFR Part 2. Your employer, your supervisor, and your HR department are not contacted as part of verification. The verification process is between Maverick and your insurance carrier directly — your employer plays no role in it.
What if my insurance denies coverage?
We work to appeal denials when there’s clinical justification, and we can talk through other options — including out-of-network coverage, payment plans, or other facilities that might be a better fit financially. We won’t pretend a denial isn’t a denial.
What if I don't have insurance?

Confidentiality and HIPAA
The information you provide on this form is protected by HIPAA (the federal Health Insurance Portability and Accountability Act) and, for substance use treatment specifically, by 42 CFR Part 2 — the most stringent federal privacy regulation governing healthcare in the United States.
Practically, this means:
- Your insurance carrier is contacted by Maverick directly. They are required to keep verification requests confidential.
- Your employer, your manager, your HR department, and your family members are not contacted unless you explicitly authorize it in writing.
- The information you submit is stored on encrypted servers and accessible only to Maverick’s admissions and clinical teams.
- If you decide not to pursue treatment with us, your information is retained per HIPAA requirements and used only for required compliance purposes.
